Folk Knowledge in a Gloucestershire Practice

Edward Jenner was born at Berkeley in Gloucestershire in 1749 and apprenticed to a country surgeon at thirteen. He completed his training in London under John Hunter, the most demanding anatomist of the age, then returned deliberately to rural practice. He was a naturalist as much as a physician, and his election to the Royal Society in 1788 rested on a study of the cuckoo, in which he described the newly hatched chick ejecting its nest-mates, an observation long doubted and later confirmed by photography.

Smallpox at that time killed a substantial share of those it infected and disfigured or blinded many who survived. Inoculation with live smallpox matter, known as variolation, offered partial protection at real risk, since inoculated patients could sicken badly and could infect others. Among dairy communities it was commonly said that those who had caught cowpox from cattle did not take the smallpox. Jenner was neither the first nor the only person to hear this, but he was the one who decided to test it.

The Experiment of 1796

In May 1796 Jenner took matter from a cowpox lesion on the hand of a dairymaid, Sarah Nelmes, and introduced it into the arm of James Phipps, the eight-year-old son of his gardener. The boy developed a mild local illness and recovered. Weeks later Jenner deliberately inoculated him with smallpox matter, and no disease followed. He repeated the challenge afterwards. The procedure is central to modern medicine and would not survive a moment's scrutiny by any present-day ethics committee. Phipps survived into adulthood, and Jenner later gave him a cottage.

He was also not first. A Dorset farmer, Benjamin Jesty, had inoculated his wife and sons with cowpox in 1774, and similar attempts are recorded in Germany. What Jesty lacked was publication. When the Royal Society declined Jenner's paper, he printed his Inquiry privately in 1798, describing case after case, and the word vaccination entered the language from vacca, the Latin for cow. Evidence gathered, argued and circulated is what turned a country observation into a method. Jesty was publicly honoured for his experiment only decades afterwards.

Spread, Resistance, and an Ending

Adoption was astonishingly fast. Within a few years the practice had reached the Americas, India and Japan, carried in part by the Spanish Balmis expedition, which transported the vaccine across the Atlantic in a chain of orphaned children infected in sequence because no other means of preservation existed. Opposition was equally vigorous. British compulsory vaccination laws in the nineteenth century provoked organised resistance, fines, imprisonment and eventually a conscientious objection clause, arguments that have never entirely gone away. Napoleon had his armies vaccinated and struck a medal in Jenner's honour.

Modern genomic work has complicated the origin story. Sequencing of nineteenth-century vaccine stocks suggests the virus long used for vaccination was closer to horsepox than to cowpox, and Jenner himself suspected an equine source he called the grease. He died at Berkeley in 1823. In 1980 the World Health Organization certified the global eradication of smallpox, the only human disease so far removed from the world, and the line back to that Gloucestershire garden is unbroken. The vaccine he started has therefore outlived the disease it was made against.
